Agentforce Alternative for Healthcare and HIPAA-Regulated Salesforce Orgs

Salesforce's BAA covers named Agentforce products but requires Trust Layer masking, which Salesforce disables for agents. GPTfy masks PHI in Salesforce before every model call and sends it to a model on your own BAA.

What is the Agentforce alternative for healthcare Salesforce orgs?

For a HIPAA-regulated Salesforce org, the deciding question is usually not whether a Business Associate Addendum exists. Salesforce's BAA lists named Agentforce products as HIPAA Covered Services. The question is whether protected health information (PHI) is masked before an AI model sees it.

Salesforce's BAA requires customers using Agentforce Services and Prompt Builder to enable Einstein Trust Layer Data Masking "to the extent within Customer's control". Salesforce also documents that LLM data masking is disabled for agents, a decision its June 2025 blog explains by latency and agent-planning accuracy.

GPTfy is a Salesforce-native alternative that keeps masking on for agents. It runs inside your org as an AppExchange managed package, masks PHI in four layers before every model call, logs every AI interaction in Salesforce, and sends the masked request to a model you contract with directly. It does not require Agentforce or Data 360.

GPTfy is a Salesforce-native Agentforce alternative on AppExchange: bring your own model (BYOM), no Data 360, fixed per-user pricing.

Does Salesforce's BAA cover Agentforce?

Named Agentforce products, yes. Salesforce's Business Associate Addendum (last updated 30 September 2026) lists, under Einstein Services, Agentforce Assistant, Agentforce Voice, Agentforce Sales Coach, Agentforce SDR, Agentforce Service Agent and Prompt Builder, with Agentforce Contact Center and Agentforce Operations listed separately. Salesforce Data 360 is also listed. Check the current list for the exact products you plan to use.

The addendum attaches conditions that matter for AI:

  • For Agentforce Services and Prompt Builder, the customer must enable, maintain and use Data Masking in the Einstein Trust Layer, to the extent within the customer's control.
  • Einstein Services may not be used as a substitute for medical advice, as medical software, or to infer or predict a person's diagnosis, condition, eligibility or outcome.
  • PHI may not be submitted to Einstein Bots utterance records.
  • When a third-party LLM provider is used, the customer is responsible for ensuring that use meets HIPAA requirements.

Is data masking on for Agentforce agents?

No. Salesforce's Trailhead module on LLM data masking states that data masking for LLMs is disabled for agents, and Salesforce's June 2025 blog says it made the decision to disable data masking for Agentforce use cases because masking added latency and hindered agent planning and actions. Masking still applies to prompt templates run directly, outside an agent.

In practice that means a healthcare team using Agentforce agents relies on the other Trust Layer controls, such as zero data retention with Salesforce's model partners, rather than on masking. Whether that meets your own HIPAA policies is a decision for your privacy and security teams. More detail: Does Einstein Trust Layer mask Agentforce data?

How does GPTfy handle PHI?

  • Masking before the model, including for agents. Four layers run inside Salesforce: field-level masking, regex pattern detection, keyword blocklists and Apex enforcement. Authorised users see de-masked responses.
  • 16 of 18 Safe Harbor identifiers. HIPAA's Safe Harbor method (45 CFR 164.514(b)(2)) lists 18 identifiers. GPTfy masks 16. It does not mask biometric identifiers, including finger and voice prints, or full-face photographic images and comparable images. Keep those out of AI prompts.
  • No GPTfy servers in the data path. GPTfy is a managed package. Requests go from your org to your model provider through a Salesforce Named Credential.
  • Audit trail in Salesforce. Every AI interaction is logged in Salesforce with the full prompt chain and the model used, and retention can be set from 30 days to indefinite with Salesforce Shield.

Masking reduces what a model sees. It is not the same as de-identification under HIPAA, which requires Safe Harbor or Expert Determination. Treat masked prompts as PHI unless your privacy officer has determined otherwise, and use a model service covered by a BAA. See data masking.

Which AI model providers sign a BAA?

With GPTfy you choose the model and hold the contract, so the model provider's BAA is yours to put in place. Status on 27 October 2026, from each provider's own pages:

ProviderBAA positionConditions to check
Amazon Bedrock (AWS)Listed as a HIPAA Eligible ServiceRequires an AWS Business Associate Addendum before PHI is used
Google Gemini Enterprise Agent Platform (formerly Vertex AI)Generative AI on Agent Platform is listed in the Google Cloud BAAVertex AI was renamed in April 2026; check the product names on Google's covered list
Anthropic APIAnthropic provides a BAA for HIPAA-ready services, including its first-party APICovered models need 30-day retention; excludes Batch, Files and beta features, among others
OpenAI APIBAA-eligible endpoints can process PHIThe account must be provisioned for a Modified Retention feature; live web search is not covered

Which option fits a healthcare org?

  • Choose Agentforce if you want Salesforce to supply the agents and models under one BAA, the specific Agentforce products you need are on its covered list, and your privacy team accepts agents running without LLM data masking.
  • Choose Prompt Builder for single-step tasks such as summaries, where Trust Layer masking stays on because no agent is involved.
  • Choose GPTfy if you want agents inside Salesforce with PHI masked before every model call, your choice of BAA-covered model provider, an audit trail in your org, and no Data 360 requirement.
  • GPTfy is not a fit if prompts must include biometric identifiers or face images, if you have fewer than 100 users, or if you want a single vendor to hold every agreement.

Key takeaways

Agentforce is on Salesforce's BAA list

Salesforce's Business Associate Addendum names Agentforce Service Agent, Agentforce Voice, Agentforce SDR, Prompt Builder and other Agentforce products as HIPAA Covered Services.

The BAA requires masking. Agents turn it off

For Agentforce Services and Prompt Builder, the BAA says the customer must enable Data Masking in the Einstein Trust Layer where it is within their control. Salesforce documents that LLM data masking is disabled for agents.

GPTfy masks before every model call

Four layers of masking run inside Salesforce before data reaches the model, covering 16 of the 18 Safe Harbor identifiers. It does not mask biometric identifiers or full-face images.

You still need a BAA with your model provider

Masked is not automatically de-identified. Run the model on a BAA-covered service: Amazon Bedrock, Google's Gemini Enterprise Agent Platform, the Anthropic API or eligible OpenAI API endpoints, each with its own conditions.

FAQ

For healthcare teams staying on Salesforce, the main options besides Agentforce are Prompt Builder used directly, where Trust Layer masking stays on; your own Apex build calling a BAA-covered model through Named Credentials; and Salesforce-native AppExchange apps such as GPTfy, which run agents inside Salesforce, mask PHI before every model call, and let you bring your own model under your own BAA.

No software is HIPAA compliant on its own; compliance depends on how a covered entity uses it. Salesforce's Business Associate Addendum lists named Agentforce products, such as Agentforce Service Agent and Agentforce Voice, as HIPAA Covered Services, on condition that the customer enables Einstein Trust Layer data masking where it is within their control. Salesforce documents that LLM data masking is disabled for agents.

GPTfy is a Salesforce-native AppExchange app that runs AI agents inside Salesforce and masks PHI in four layers before every model call, including for agents. It sends masked requests to a model on your own contract, such as Amazon Bedrock, Google's Gemini Enterprise Agent Platform or the Anthropic API, each of which offers a BAA with conditions. It does not require Agentforce or Data 360.

No. GPTfy masks 16 of the 18 Safe Harbor identifiers. It does not mask biometric identifiers, such as finger and voice prints, or full-face photographic images, so those should be kept out of AI prompts.

Yes. Masking reduces what the model sees but is not de-identification under HIPAA, so treat masked prompts as PHI unless your privacy officer decides otherwise. Use a model service covered by a BAA and check that provider's conditions, such as retention settings and excluded features.

No. Salesforce's Trailhead states that data masking for LLMs is disabled for agents. Masking still applies to prompt templates run directly in Prompt Builder or Flow.
