GPTfy - Salesforce Native AI Platform

Care Plans. Clear at a Glance.

GPTfy replaces truncated Health Cloud care plan views with AI-generated tables showing full task titles, goal definitions, and status — all in one place.

Truncated task titles and cut-off goal definitions in Health Cloud care plan records force coordinators to click into every item just to understand what it says. GPTfy generates a unified care plan view — complete task names, goal definitions, and status — directly on the record page, with patient data anonymized before AI processing.

What this demo covers

Unified Care Plan View

  • Goal assignments and tasks are presented together in a single, readable landing page.
  • Coordinators see the full care plan at a glance without clicking into individual records.

Full Title and Status Visibility

  • Task titles and goal definitions are displayed in full — no truncation.
  • Task status is included in the AI output, surfacing information hidden by the native layout.

PII Anonymization Before AI

  • Care plan data is anonymized within Salesforce before being sent to the AI provider.
  • Raw patient data stays in your org — only sanitized content reaches the model.

Flow Integration for Automation

  • The clear task and goal view connects to Salesforce Flow for assignment automation.
  • Unassigned or overdue tasks can trigger escalation notifications without manual review.

Use this video when

A care coordinator reviews an active care plan and needs to see all task statuses without clicking into each record

A supervisor audits multiple care plans and uses the unified view to identify incomplete or overdue tasks

A new team member picks up an unfamiliar patient's care plan and needs to understand its structure immediately

A clinical manager reviews goal assignments across a program to confirm they align with the care plan objectives

An admin connects care plan task status to a Flow automation that reassigns overdue tasks automatically

A care team prepares for a patient review meeting and pulls the unified care plan summary as a briefing document

Frequently asked questions

In the native Health Cloud care plan record layout, task titles and goal definitions are often truncated — cut off mid-title because of column width constraints. Care coordinators cannot see the full name of a task or goal without clicking into each individual record, which slows down case reviews and creates confusion about what each item is trying to accomplish.

GPTfy runs a Care Plan Summary prompt against the record, anonymizes the data, and sends it to AI. The AI returns two structured tables — one for goal assignments and one for tasks — with full titles, descriptions, and status fields displayed clearly. This replaces the truncated native view with a complete, unified landing page for the care plan.

The goal assignments table shows the full goal definitions and their current status. The task table shows complete task titles along with their status — information that is not visible in the truncated native layout. Both tables appear together in a unified view at the top of the care plan record page.

Yes. GPTfy anonymizes personally identifiable information before any care plan data leaves Salesforce. Patient identifiers are stripped within the org before the sanitized content is sent to the AI provider. The raw care plan data remains in your Health Cloud environment.

The AI summary surfaces task status and goal assignments clearly, making it faster for coordinators to identify unassigned or overdue tasks. This view can also be connected to Salesforce Flow automations that trigger reassignment logic or escalation notifications based on task status — enabling automated assignment workflows without custom code.

Care coordinators who manage multiple active care plans benefit most from the unified summary view, as it reduces the time spent clicking into individual task and goal records. Clinical supervisors use the view to audit care plan completeness across a caseload, and new team members use it to quickly understand an unfamiliar patient's care plan structure.

Video transcript
Now we're going to use AI to give us a more cohesive view of a care plan record page. Here we have the goal assignments, and here we have the tasks associated with the care plan. We're going to run the Care Plan Summary prompt. It's going to anonymize the data, send it over to AI, and it's going to return two tables showing the goal assignments and the tasks in a more cohesive and unified view. Because the problem with this current view is that the titles of the tasks are cut off. And we don't know much about these tasks beyond these fractions of titles — such as their status, for example. And with the goal assignments, once again, the goal definitions are cut off, and all of these cut-off titles can lead to user confusion as they might not get a full picture of what these are trying to say. So it's going to take a few more seconds for AI to process this information and give us this improved view of the record page. And there we go. Now the care plan record page has been turned into a more cohesive landing page with your tasks and goal assignments laid out very clearly in front of you. And once again, it gives you more information beyond what is given in the native view — such as the status of the tasks.
