Care coordinators cannot see the full name of a task or goal without clicking into each individual record, which slows down case reviews and creates confusion about what each item is trying to accomplish.

GPTfy runs a Care Plan Summary prompt against the record, anonymizes the data, and sends it to AI. The AI returns two structured tables — one for goal assignments and one for tasks — with full titles, descriptions, and status fields displayed clearly. This replaces the truncated native view with a complete, unified landing page for the care plan.
